09/10/2025 – PROBATIONARY REQUEST - Doctor’s request for approval to remove the monitoring physician and chart review requirement contained in the 8/9/23 Board Order, approved by vote of the Board on 9/10/25. <br/> <br/>01/10/2024 - PROBATIONARY REINSTATMENT - Request for reinstatement of license to practice osteopathic medicine and surgery approved, and subject to probationary terms and conditions as outlined in the 8/10/23 Order for at least three years. <br/> <br/>08/09/2023 -BOARD ACTION - Application for license to practice osteopathic medicine and surgery granted, and immediately suspended for an indefinite period of time, and subject to conditions for reinstatement including completion of a Post-Licensure Assessment System, and probationary terms conditions and limitations for at least three years. Based on the submission of an application to practice osteopathic medicine and surgery, and on the finding that the doctor’s prior disciplinary history including a 8/13/14 Notice and a 1/13/16 Order issued by the State Medical Board of Ohio, and prior actions by: the Texas Medical Board which issued a 7/19/02 Temporary Suspension Order and a 6/8/04 Revocation Order; and the Michigan Department of Community Health, Bureau of Health Professions which issued a 7/21/04 Order of Summary Suspension, a Consent Order adopted on 2/3/05, and a 10/28/05 Final Order Granting Reinstatement; and the West Virginia Board of Osteopathy which issued a 9/1/04 Consent Agreement and Order; and also based on the doctor’s DEA registrations being suspended and/or terminated in Texas, and West Virginia; and on the doctor’s Medicaid provider agreement being suspended by the Ohio Department of Medicaid which individually and/or collectively constitute: any of the following actions taken by an agency responsible for authorizing, certifying, or regulating an individual to practice a health care occupation or provide health care services in this state or another jurisdiction, for any reason other than the nonpayment of fees: the limitation, revocation, or suspension of an individual's license to practice; acceptance of an individual's license surrender; denial of a license; refusal to renew or reinstate a license; imposition of probation; or issuance of an order of censure or other reprimand; and the revocation, suspension, restriction, reduction, or termination of clinical privileges by the United States department of defense or department of veterans affairs or the termination or suspension of a certificate of registration to prescribe drugs by the drug enforcement administration of the United States department of justice; and the termination or suspension from participation in the Medicare or Medicaid programs by the department of health and human services or other responsible agency. (Journal Entry-No Hearing Requested) <br/> <br/>12/10/2014 PARTIAL DISMISSAL - By Order and Entry dated 12/10/14, allegations contained in the 08/13/14 Notice of Opportunity for Hearing alleging violations based on the summary suspension issued by the West Virginia Board of Osteopathic Medicine, is dismissed without prejudice. The portions of the Notice of Opportunity for Hearing that are based on the prior Texas Medical Board and Michigan Department of Community Health, Bureau of Health Professions remains in effect. Effective 12/11/14. <br/> <br/>08/13/2014: CITATION - Based on prior action against the doctor’s license by the Texas Medical Board which revoked the doctor’s license based on findings that he failed to practice medicine in an acceptable professional manner in regard to three patients; prior action against the doctor’s license by the Michigan Department of Community Health, Bureau of Health Professions; and prior action by the West Virginia Board of Osteopathic Medicine which summarily suspended the doctor’s license based on its findings that probable cause exists to believe that the doctor engaged in unprofessional conduct and conduct, practices and acts that constitutes a department from accepted standards of professional conduct.
