eLicense Ohio (eLicense.Ohio.gov) is a service provided by the Ohio Department of Administrative Services (DAS) on behalf of a number of State professional licensing agencies, boards and commissions. The State recognizes the importance of privacy to our website visitors. Therefore, visitors should be aware of the following information. This statement discloses to our website visitors what personal information is being collected on this website and how that personal information may be used.
Visitors should also be aware of any Privacy Notices posted on the web sites of the licensing agency, board or commission they are conducting business with. Those web sites can be found on the Contact page.
Visitors are also expected to know and agree to the General Terms .
What We Collect
The eLicense Ohio service collects the following information from you on behalf of one or more licensing agencies, boards and commissions with whom you are conducting business:
- Name,
- Address,
- Date of birth,
- Social Security Number - Your Social Security number is required for accurate identification and reporting under federal and state child support enforcement law (42 U.S.C. Sec. 666 and ORC 3123.50). Your Social Security number may also be used by your licensing agency(s), board(s) or commission(s) for purposes of reporting to national organizations, law enforcement authorities for investigative/law enforcement purposes, and/or as otherwise required by state and federal law.
- Military service,
- eMail address, and
- Demographic and workforce information is collected for some licensed healthcare professions.
We also collect Generic Internet-user information. This may include:
- Internet protocol address of the individual computer (or proxy server) making the request,
- Type of internet browser or computer operating system used to access the site,
- Date and time site was accessed,
- Webpages visited on this site, and
- Internet address of the site from which you may have linked to this site.
Information and payments submitted through eLicense Ohio require authentication (logging in with a username and password), and are submitted through a secure Internet connection.
What We Do With the Information We Collect
Personally identifiable information submitted through the eLicense Ohio service is used by your licensing agency, board or commission to process requests, issue and administer licenses.
Information submitted through email is used by your licensing agency, board or commission to answer or process user requests as effectively as possible.
Information about your military service is used by your licensing agency, board or commission to determine if you or your spouse are eligible for certain benefits under the Ohio licensing laws, rules or policies.
Demographic and workforce data collected for some licensed healthcare professions is used to enhance the state’s capacity for healthcare workforce forecasting, policy development, and research. This data is used to analyze the supply and demand of the healthcare workforce serving Ohio.
All information collected through server log files is used in aggregate form to analyze usage trends and statistics to help the State of Ohio improve its web services. It may also be used to identify and respond to threats to the State network.
Cookies may be used when necessary to complete a transaction, to process data submitted online to the State, or to facilitate ongoing Internet interactions
Public Records
Public disclosure of records, and therefore the records associated with the use of eLicense Ohio are governed by statutes and the privacy policy of the board or commission that holds legal administrative authority over the information and payments submitted. Information submitted via the eLicense Ohio portal is not necessarily a public record. Likewise, only the board or commission that holds legal administrative authority over the information can address public records requests.
Visitors should be aware that, in general, most documents and records maintained by state of Ohio agencies and organizations, including electronic information, are public records under Ohio law. Therefore information collected or submitted through your visit to this website, may be subject to a public records request.

State guidelines are followed to appropriately protect all information systems under our control.
Access and Dispute of Information
Since the records collected in eLicense Ohio are on behalf of participating agencies, boards and commissions, all requests by individuals to review or dispute their own information must be directed to those agencies, boards or commissions.
