Board Action Details On May 12, 2025, based on allegations CRE8 Pharmacy Group, a non-resident pharmacy, engaged in non-approved drug compounding and shipping into Ohio, the Ohio Board of Pharmacy (Board) issued a Summary Suspension/Notice of Opportunity for Hearing. On July 10, 2025, CRE8 Pharmacy Group, LLC entered into a Settlement Agreement with the Board in which they agreed to pay a monetary penalty in the amount of $200,000.
