Board Action Details On May 4, 2021, based on allegations that Mercy Hospital Anderson Pharmacy’s RP Jason Glasgow, stated Nathan Shaw was performing pharmacy technician duties without obtaining proper registration with the Board, the State of Ohio Board of Pharmacy (Board) issued a Notice of Opportunity for Hearing. On June 10, 2021, Mercy Hospital Anderson Pharmacy entered into a settlement agreement with the Board in which they agreed to pay a monetary penalty of $500.00.
