Board Action Details On March 6, 2024, based on allegations that Lap Pan Chu failed to maintain minimum standards and received discipline from another State Board of Pharmacy, the Ohio Board of Pharmacy (Board) issued a Notice of Opportunity for Hearing. On November 4, 2024, Lap Pan Chu entered into a Settlement Agreement with the Board in which they agreed to a monetary penalty in the amount of $5000, completion of 1.2 CEUs, attendance of a Responsible Person Roundtable and a five-year probationary period. <br/> <br/>On June 25, 2021, based on allegations that Lap Pan Chu failed to complete Board Ordered continuing pharmacy education the State of Ohio Board of Pharmacy (Board) issued a Notice of Opportunity for Hearing. On November 16, 2021, the Board issued an Order requiring continuing pharmacy education and a monetary penalty. <br/> <br/>On April 22, 2020, based on allegations that Lap Pan Chu allowed an employee to perform the duties of a pharmacy technician without obtaining appropriate registration with the Board, the State of Ohio Board of Pharmacy (Board) issued a Notice of Opportunity for Hearing/Proposal to take Disciplinary Action Against Licensee. On June 9, 2020, Lap Pan Chu entered into a settlement agreement with the Board in which he agreed to pay a monetary penalty in the amount of $1,000.00 and to take continuing education classes.
