Board Action Details On September 28, 2023, based on allegations that Kroger Pharmacy #522 failed to maintain drug security of their controlled substances and as a result experienced drug losses, the State of Ohio Board of Pharmacy (Board) issued a Notice of Opportunity for Hearing. On November 16, 2023, Kroger Pharmacy #522 entered into a Settlement Agreement with the Board in which they agreed to pay a monetary penalty in the amount of $1000.00.
