Board Action Details On December 22, 2022, based on allegations that CVS Pharmacy #4101 failed to maintain the minimum standards of a Terminal Distributor of Dangerous Drugs, the State of Ohio Board of Pharmacy (Board) issued a Notice of Opportunity for Hearing.
