Board Action Details On October 13, 2022, based on allegations that Blanchard Valley Hospital sold dangerous drugs to an unlicensed Terminal Distributer of Dangerous Drugs, the State of Ohio Board of Pharmacy (Board) issued a Notice of Opportunity for Hearing. On March 15, 2023, Blanchard Valley Hospital entered into a Settlement Agreement with the Board in which they agreed to pay a monetary penalty in the amount of $1,125.00.
