Board Action Details On August 24, 2021, based on allegations that Benjamin Prewitt errored in dispensing a prescription, the State of Ohio Board of Pharmacy (Board) issued a Notice of Opportunity for Hearing. On January 4, 2022, Benjamin Prewitt entered into a Settlement Agreement with the Board in which he agreed to a monetary penalty in the amount of $500.00 and to obtain approved continuing pharmacy education.
