Board Action Details On September 3, 2021, and on September 15, 2022, based on allegations that Kroger Pharmacy #014447 had been issued multiple warnings due to (but not limited to) drug losses and drug security issues, the State of Ohio Board of Pharmacy (Board) issued a Notice of Opportunity for Hearing. On December 15, 2022, the Board issued two Orders, each imposing a monetary penalty of $6,000.00 and a requirement that the Responsible Person must attend the “Responsible Person Roundtable” .
