Board Action Details On February 9, 2022, based on allegations that TWC Pharmacy and Wellness Center, LLC failed to conduct annual controlled substance inventory since approximately December <br/>2015 and had discrepancies in their inventory, the State of Ohio Board of Pharmacy (Board) issued a Notice of Opportunity for Hearing. On April 21, 2022, TWC Pharmacy and Wellness Center, LLC entered into a settlement agreement with the Board in which they agreed to a permanently and voluntary surrender effective the date of their discontinuation of business, March 27, 2022.
