Board Action Details On May 30, 2025, based on allegations that CVS/Pharmacy #6139 failed to report a drug theft and failed to cooperate with a Board investigation, the Ohio Board of Pharmacy (Board) issued a Notice of Opportunity for Hearing. On July 23, 2025, CVS/Pharmacy #6139 entered into a Settlement Agreement with the Board in which they agreed to pay a monetary penalty in the amount of $500.
