Board Action Details On December 10, 2021, based on allegations that Clark LowCost Pharmacy stored the Moderna COVID-19 vaccine outside the acceptable temperature range, the State of Ohio Board of Pharmacy (Board) issued a Notice of Opportunity for Hearing. On April 14, 2022, Clark LowCost Pharmacy entered into a settlement agreement with the Board in which they agreed to pay a monetary penalty in the amount of $1,000.00.
