On October 18, 2024, based on allegations that CVS/Pharmacy #4401 provided the Board with a DEA-106 form, confirming a loss of #424 1mg alprazolam tablets and #389 2mg alprazolam tablets, the Ohio Board of Pharmacy (Board) issued a Notice of Opportunity for Hearing.
