Board Action Details As of December 22, 2022 licensee has successfully been released from probation and the behavioral health agency per the terms of her Consent Agreement (Consent). Per her request, her five-year stayed suspension is hereby amended and has ended as of December 22, 2022. See letter dated December 22, 2022 attached to her Consent. <br/> <br/>Licensee is still required to comply with the remaining terms of her Consent Agreement, e.g., passing her continuing education (CE) audit verifying completion of her hours for the 2021-2022 and 2023-2024 practice biennium. Her case number will remain open while monitoring her future CE Audits. Her adverse action report with the National Practitioner Data Bank has been updated. Below are the original terms of her Consent. <br/> <br/>Licensee answered "Yes" on her 2021-22 renewal application to a question (that required disclosure) regarding she plead guilty, was convicted of DUI, an M1, in 2020. Consent Agreement (Consent) terms: Suspended for five years and stayed as long is in compliance with the terms; Reprimanded; Complete 2.5 hours of stress related continuing education (CE); Audit CE hours for three practice bienniums; Provide a release and information regarding the following 3: 1) Criminal Probation 2) Behavioral Health Agency (Agency) treatment records 3) Alcoholics Anonymous; Complete any and all programs required by the court and/or her Probation; Submit a quarterly report from the Agency. Each quarterly report shall opine that she is fit to practice and perform her job as a speech-language pathologist; After release from Probation and the Agency or any time prior to the stayed suspension end date may apply to have the five-year stayed suspension amended; Report any new conviction and/or if she has resumed treatment.
