Board Action Details 1997 <br/>Pursuant to an Adjudication Order, her speech-language pathology (SLP) license was Revoked September 5, 1997 for not completing the required continuing education (CE) hours for the 1995-1996 practice biennium. Although she had continued to renew her license up until this time, she had not practiced as a SLP since 1987 (over 18 years). <br/> <br/>2011 <br/>Licensee applied for reinstatement February 2, 2011 along with proof of completion of 40 hours of CE. All hours were obtained on-line. Having not practiced as an SLP since 1987, pursuant to a Consent Agreement (Consent) effective June 22, 2011, reinstatement was granted with the following conditions: Complete 10 additional hours of CE, within 12 months, via in-person (conferences, workshops). The hours shall be specific to the clinical practice of SLP; Once employment gained, complete a 1-year mentorship being supervised face to face. Direct supervision shall be as follows: Quarter 1 - 25%, Quarter 2 - 20%, Quarter 3 - 15%, Quarter 4 - 10%. At the end of each quarter, shall submit a Report and Log (Log) verifying completion of supervision. The Logs received did not demonstrate she was employed at least 15 hours per week during her 1-year mentorship. She was employed a total of 184 hours during the mentorship. Effective August 6, 2013, Addendum to the 2011 Consent terms were: within 18 months, shall prove she was employed a total of 1,080 hours. Once the hours are accumulated prior to the 18th month, the Addendum to the 2011 Consent will terminate. Addendum was terminated December 3, 2013 due to she satisfied the terms of the Addendum.
