Based on doctor’s admission of having pled guilty to three felony charges of Obtaining a Schedule II Controlled Substance by Fraud; having been diagnosed with opiate dependence and alcohol abuse, for which doctor was reprimanded by the Virginia licensing board; having completed the terms of a Consent Order with the Virginia board; and having been deemed capable of practicing according to acceptable and prevailing standards of care so long as certain treatment and monitoring conditions are in place. Agreement effective 7/14/10; agreement to remain in effect for at least three years prior to any request for termination.
