(Non-disciplinary) <br/> <br/> 03/11/2009: REQUEST FOR RECONSIDERATION: Doctor’s request for reconsideration of the Board’s 1/14/09 Order, which stayed the permanent revocation of doctor’s medical license subject to suspension for at least one year and established interim monitoring conditions, conditions for reinstatement, and subsequent probationary terms and conditions, denied by Board vote on 3/11/09. <br/> <br/> 01/14/2009: BOARD ORDER: Permanent revocation of medical license stayed subject to suspension for at least one year; interim monitoring conditions, conditions for reinstatement, and subsequent probationary terms, conditions, and limitations for at least five years established. Based on Board’s determination that a Pennsylvania judicial finding concerning the doctor is substantially equivalent to intervention in lieu of conviction for acts that constitute a felony in Ohio; doctor is impaired in ability to practice according to acceptable and prevailing standards of care because of habitual or excessive use or abuse of drugs; and doctor’s physical and psychiatric disorders constitute an inability to practice according to acceptable and prevailing standards of care by reason of mental illness or physical illness. Order effective 2/13/09. <br/> <br/> 05/14/2008: CITATION:Based on doctor having received a judicial finding in the Court of Common Pleas of Crawford County, Pennsylvania that is substantially equivalent to intervention in lieu of conviction for a misdemeanor committed in the course of practice and on doctor’s alleged commission of an act that constitutes a felony. The underlying basis being doctor’s writing a prescription for Adderall in the name of another person when the drug was for doctor’s own use. Notice of opportunity for hearing mailed 5/15/08.
