Release effective immediately. <br/> <br/> 04/12/2023 - PROBATIONARY MODIFICATION - Doctor’s request for approval of medical records course, granted by vote of the Board on 4/12/23. <br/> <br/>12/14/2022 CONSENT AGREEMENT - License to practice allopathic medicine and surgery reprimanded, and subject to probationary terms and conditions requiring completion of educational courses, and payment of a five thousand dollar ($5,000.00) fine. Based on the doctor’s admission that when he entered into a 5/26/22 Consent Order with the West Virginia Board of Medicine which reprimanded his license for prescribing to failing to document prescriptions written to family members, those acts and/or omissions individually and/or collective constitute: any of the following actions taken by an agency responsible for authorizing, certifying, or regulating an individual to practice a health care occupation or provide health care services in this state or another jurisdiction, for any reason other than the nonpayment of fees: the limitation, revocation, or suspension of an individual’s license to practice; acceptance of an individual’s license surrender; denial of a license; refusal to renew or reinstate a license; imposition of probation; or issuance of an order of censure or other reprimand. Agreement effective 12/14/22.
