03/09/2022 BOARD ORDER - License to practice allopathic medicine and surgery indefinitely suspended, and subject to a three thousand dollar ($3,000.00) fine, and conditions for reinstatement. Based on the finding that when the doctor received an Exclusion Notice by the United States Department of Health and Human Services; failed to provide requested patient records to the Board, and made statements contrary to his signed certification; and a prior action by the Maryland State Board of Physicians, which issued a 4/20/2020 Final Decision and Order, those actions individually and/or collectively constitute: the termination or suspension from participation in the Medicare or Medicaid programs by the Department of Health and Human Services or other responsible agency; a failure to cooperate in an investigation conducted by the board, including failure to comply with a subpoena or order issued by the board or failure to answer truthfully a question presented by the board in an investigative interview, an investigative office conference, at a deposition, or in written interrogatories, except that failure to cooperate with an investigation shall not constitute grounds for discipline under this section if a court of competent jurisdiction has issued an order that either quashes a subpoena or permits the individual to withhold the testimony or evidence in issue; and any of the following actions taken by an agency responsible for authorizing, certifying, or regulating an individual to practice a health care occupation or provide health care services in this state or another jurisdiction, for any reason other than the nonpayment of fees: and the limitation, revocation, or suspension of an individual's license to practice; acceptance of an individual's license surrender; denial of a license; refusal to renew or reinstate a license; imposition of probation; or issuance of an order of censure or other reprimand. Order effective 3/23/22.

07/08/2020 CITATION - Based on a prior action by the Maryland State Board of Physicians, which issued a 4/20/2020 Final Decision and Order, that found the doctor had failed to cooperate with an investigation and that the doctor was professionally and mentally incompetent to practice medicine, which as alleged individually and/or collectively, constitute any of the following actions taken by an agency responsible for authorizing, certifying, or regulating an individual to practice a health care occupation or provide health care services in this state or another jurisdiction, for any reason other than the nonpayment of fees: the limitation, revocation, or suspension of an individual's license to practice; acceptance of an individual's license surrender; denial of a license; refusal to renew or reinstate a license; imposition of probation; or issuance of an order of censure or other reprimand. 09/11/19 CITATION - Based on allegations that the doctor was excluded from participation in Medicare, Medicaid, and all Federal health care programs by the United States Department of Health and Human Services because of the doctor’s failure to cooperate in an audit. Also based on: the doctor’s response to a Board subpoena, to which he provided eleven of the requested thirteen patient records; and on the doctor’s statement during a board deposition that he was unsure as to the accuracy or completeness of the provided records, which was contrary to his signed certification. 02/14/18 PROBATION TERMINATED - Doctor’s request for release from the terms of the 2/8/2017 Consent Agreement granted by vote of the Board on February 14, 2018. Effective immediately.

12/13/17 PROBATIONARY REQUEST - Doctor’s request for approval of a controlled substance prescribing course, granted by vote of the Board on 12/13/2017.

02/08/2017: CONSENT AGREEMENT - License to practice medicine and surgery reprimanded and subject to probationary terms, conditions and limitations for at least one year. Based on prior action by the Maryland State Board of Physicians which issued a reprimand on the doctor’s license for pre-signing Schedule II Controlled Substance prescription forms. Agreement effective 2/8/17.
