CLICK ON “YES” OR CONTACT LEGAL SERVICES OFFICE. <br/> <br/>07/25/19 COURT ACTION - Notice of Appeal filed 7/25/2019, in the Court of Common Pleas, Franklin County, Ohio. <br/> <br/>07/10/19 BOARD ORDER - On the thirty-first day following the effective date of the order, the doctor’s license to practice osteopathic medicine and surgery is permanently revoked and fined three thousand five hundred dollars ($3,500.00). Based on the finding that with respect to the care of eight patients, the doctor failed to adequately assess, examine, treat or properly document various psychiatric disorders by not utilizing data from the Ohio Automated Rx Reporting System, and thereby the doctor failed to maintain minimal standards applicable to the selection or administration of drugs, or failed to employ acceptable scientific methods in the selection of drugs or other modalities for treatment of disease; and that the doctor departed from or failed to conform to minimal standards of care of similar practitioners under the same or similar circumstances; and that the doctor violated or attempted to violate, directly or indirectly, or assisted in or abetted the violation of, or conspired to violate Section 4731.22(B)(20), Ohio Revised Code to wit: General Provisions, Rule 4731-11-02, Ohio Administrative Code. Based on Board’s determination that doctor committed an act constituting a misdemeanor in the course of practice and departed from the minimal standards of care applicable to the selection or administration of drugs and the minimal standards of care of similar practitioners, the underlying basis being doctor having pre-signed otherwise blank prescriptions prior to their issuance and permitting staff members who are not legally authorized to prescribe to complete the pre-signed prescriptions for dangerous drugs and/or controlled substances. Order effective 9/1/10. Suspension effective 9/2/10 through 11/30/10. <br/> <br/> 07/08/2009: CITATION: Based on doctor’s alleged removal and destruction of documentation in a patient record subpoenaed by the Board, an act that constitutes a felony, and alleged pre-signing blank prescriptions and permitting office staff who are not legally authorized to prescribe to complete prescriptions for dangerous and/or controlled substances from a supply of pre-signed blank prescriptions.
