Board Action Details 04/09/2025 - PERMANENT SURRENDER AND REVOCATION - Permanent surrender and revocation of license to practice medicine and surgery, authorized by doctor in lieu of further formal disciplinary proceedings pursuant to Sections 4731.22(B)(26), and 4731.22(B)(5) Ohio Revised Code, and as set forth in the Notices of Opportunity for Hearing issued by the Board on 12/11/24, and 2/12/25. <br/> <br/>2/12/2025 - NOTICE OF OPPORTUNITY FOR HEARING - Based on allegations when the doctor failed to disclose a 07/29/24 OVI conviction when he submitted a 03/22/15 renewal application, and failed to disclose a subsequent 10/01/18 OVI conviction when he submitted 03/25/19 renewal application, those acts and/or omissions individually and/or collectively constitute: making a false, fraudulent, deceptive, or misleading statement in the solicitation of or advertising for patients; in relation to the practice of medicine and surgery, osteopathic medicine and surgery, podiatric medicine and surgery, or a limited branch of medicine; or in securing or attempting to secure any license or certificate to practice issued by the Board. <br/> <br/>12/11/2024 - NOTICE OF OPPORTUNITY - Based on allegations when the doctor was determined to be actively impaired following a 72-hour inpatient examination and found to be unfit to continue the practice of medicine and surgery, those acts and/or omissions individually and/or collectively constitute: impairment of ability to practice according to acceptable and prevailing standards of care because of substance use disorder or excessive use or abuse of drugs, alcohol, or other substances that may impair ability to practice. <br/> <br/>12/11/2024 - PRE-HEARING SUSPENSION - Pursuant to Section 4731.22(G), Ohio Revised Code, license to practice medicine and surgery summarily suspended based on Board’s determination that there is clear and convincing evidence that the licensee is in violation of Section 4731.22(B)(26), Ohio Revised Code, and that his continued practice presents a danger of immediate and serious harm to the public.
