Release effective immediately. <br/> <br/>05/08/2024 CONSENT AGREEMENT - License to practice osteopathic medicine and surgery reprimanded, with imposed conditions including successful completion of educational courses and payment of a three thousand five hundred dollar ($3,500.00) fine. Based on the doctor’s admission that when he failed to consistently document checking OARRS or the required patient information and/or tools available for monitoring patient usage of stimulants or other controlled substances, or consistently obtain thorough patient medication history, considerations, or use available screening or diagnostic tools, those actions and/or omissions, individually and/or collectively constitute: violating or attempting to violate, directly or indirectly, or assisting in or abetting the violation of, or conspiring to violate, any provisions of this chapter or any rule promulgated by the Board, to wit: Standards and Procedures for Review of Ohio Automated Rx Reporting System, and Utilization of Anabolic Steroids, Schedule II Controlled Substance Cocaine Hydrochloride, and Schedule II Controlled Substance Stimulants. Agreement effective 5/8/24.
