12/10/25 – CONSENT AGREEMENT - License to practice osteopathic medicine and surgery indefinitely suspended, but not less than one hundred eighty (180) days, and subject to interim monitoring requirements including the completion of educational courses and the payment of a three thousand dollar ($3,000.00) fine, and conditions for reinstatement. Based on the doctor’s admission with respect to the care and treatment of eight identified patients, when the doctor inappropriately treated, failed to appropriately treat, and/or failed to appropriately document treatment with respect to inappropriate prescribing, failed to appropriately monitor patient medications, or provide appropriate care, or manage patient conditions, or incomplete documentation, those acts and/or omissions individually and/or collectively constitute: a departure from, or the failure to conform to, minimal standards of care of similar practitioners under the same or similar circumstances, whether or not actual injury to a patient is established; a failure to maintain minimal standards applicable to the selection or administration of drugs, or failure to employ acceptable scientific methods in the selection of drugs or other modalities for treatment of disease; and violating or attempting to violate, directly or indirectly, or assisting in or abetting the violation of, or conspiring to violate, any provisions of this chapter or any rule promulgated by the Board, to wit: General Provisions; Utilization of schedule II controlled substance stimulants; and Standards and Procedures for Review of “Ohio Automated Rx Reporting System.” <br/> <br/>12/11/2024 - NOTICE OF OPPORTUNITY - Based on allegations with respect to the care and treatment of eight identified patients, when the doctor inappropriately treated, failed to appropriately treat, and/or failed to appropriately document treatment with respect to inappropriate prescribing, failing to appropriately monitor patient medications, failing to provide appropriate care, failing to manage patient conditions, or incomplete documentation, those acts and/or omissions individually and/or collectively constitute: failure to maintain minimal standards applicable to the selection or administration of drugs, or failure to employ acceptable scientific methods in the selection of drugs or other modalities for treatment of disease; departure from, or the failure to conform to, minimal standards of care of similar practitioners under the same or similar circumstances, whether or not actual injury to a patient is established; violating or attempting to violate, directly or indirectly, or assisting in or abetting the violation of, or conspiring to violate, any provisions of this chapter or any rule promulgated by the Board, to wit: General Provisions, Utilization of schedule II controlled substance stimulants; and Standards and Procedures for Review of “Ohio Automated Rx Reporting System.”
