Board Action Details 09/14/2023 -FORMAL ACTION -License to practice medicine and surgery reprimanded, permanently limited and restricted that doctor shall be precluded from the evaluation, diagnosing and treatment of any patient for polycythemia, and subject to payment of a fine in the amount of three thousand five hundred dollar ($3500.00). Based on the finding that a prior disciplinary action by the New York State Department of Health, Board for Professional Medical Conduct, which issued a Statement of Charges, and a subsequent Consent Agreement, those acts and/or omissions individually and/or collectively constitute: any of the following actions taken by an agency responsible for authorizing, certifying, or regulating an individual to practice a health care occupation or provide health care services in this state or another jurisdiction, for any reason other than the nonpayment of fees: the limitation, revocation, or suspension of an individual’s license to practice; acceptance of an individual’s license surrender; denial of a license; refusal to renew or reinstate a license; imposition of probation; or issuance of an order of censure or other reprimand.
