Board Action Details 10/8/25 – CONSENT AGREEMENT: License to practice medicine and surgery reinstated subject to probationary terms, conditions and limitations for at least two years. Based on doctor having completed all requirements for reinstatement of license pursuant to the terms of a 6/8/22 consent agreement. <br/> <br/>02/05/2024 - BOARD ORDER - License to practice allopathic medicine and surgery reprimanded, and subject to payment of a one thousand dollar ($1,000.00) fine, and all terms and conditions contained in the 6/8/22 Consent Agreement to remain in effect. Based on the finding when the doctor executed a Surrender for Cause of DEA Certificate of Registration [DEA Surrender], to the U.S. Department of Justice – Drug Enforcement Agency; and on a prior disciplinary action by the Department of Financial and Professional Regulation of the State of Illinois which entered into a Consent Agreement, those acts and/or omissions individually and/or collectively constitute: revocation, suspension, restriction, reduction, or termination of clinical privileges by the United States department of defense or department of veterans affairs or the termination or suspension of a certificate of registration to prescribe drugs by the drug enforcement administration of the United States department of justice; and any of the following actions taken by an agency responsible for authorizing, certifying, or regulating an individual to practice a health care occupation or provide health care services in this state or another jurisdiction, for any reason other than the nonpayment of fees: the limitation, revocation, or suspension of an individual’s license to practice; acceptance of an individual’s license surrender; denial of a license; refusal to renew or reinstate a license; imposition of probation; or issuance of an order of censure or other reprimand; and violation of the conditions of limitation placed by the board upon a license or certificate to practice.
