Based on the doctor’s admission that when the Nevada Medical Board issued a 10/3/2017 Consent Agreement reprimanding the doctor’s license for failing to cooperate with the Nevada Medical Board during an investigation, that action constitutes any of the following actions taken by an agency responsible for authorizing, certifying, or regulating an individual to practice a health care occupation or provide health care services in this state or another jurisdiction, for any reason other than the nonpayment of fees: the limitation, revocation, or suspension of an individual's license to practice; acceptance of an individual's license surrender; denial of a license; refusal to renew or reinstate a license; imposition of probation; or issuance of an order of censure or other reprimand. Agreement effective 2/13/2019. <br/> <br/>10/10/18 CITATION - Based on prior action by the Board of Medical Examiner of the State of Nevada, which issued a June 4, 2018 Consent Agreement in which the doctor was reprimanded and ordered to pay a fine of one thousand two hundred ($1,200.00) dollars and to pay costs in the amount of one thousand eight hundred fifty-two dollars and 83/100 cents ($1,852.83). Based on the doctor’s admission that he failed to make medical records available to a patient despite repeated requests from the patient; and that he failed to comply with a lawful order issued by the Nevada Board.
