Release effective immediately. <br/> <br/>05/08/2024 - PROBATIONARY REQUEST - Doctor’s request for approval of a thirty (30) day extension to complete required educational courses, granted by vote of the Board on 5/8/24. <br/> <br/>04/12/2023 -CONSENT AGREEMENT-License to practice allopathic medicine and surgery reprimanded, and subject to probationary terms including completion of educational courses, and payment of a five thousand dollar ($5,000.00) fine. Based on the doctor’s admission that when he pre-signed prescriptions, those acts and/or omissions individually and/or collectively constitute: the commission of an act in the course of practice that constitutes a misdemeanor in this state, regardless of the jurisdiction in which the act was committed, to wit: Authority of Licensed Health Professional. Agreement effective 4/12/23.
