Agreement effective 1/10/24. <br/> <br/>05/12/2021 PROBATIONARY REQUEST - Physician assistant’s request for approval of reporting physician, granted by vote of the Board on 5/12/21. <br/> <br/>02/10/2021 PROBATIONARY REQUEST - Physician assistant’s request for approval of reporting physician, granted by vote of the Board on 2/10/21. <br/> <br/>08/12/2020 PROBATION MODIFIED - Physician Assistant’s request for approval of a reduction in personal appearances from every three to every six months; and approval of a reduction in recovery meeting attendance, granted by vote of the Board on 8/12/2020. <br/> <br/>07/10/19 CONSENT AGREEMENT - Physician assistant’s license to practice reinstated, and subject to probationary terms, conditions and limitations for at least five years. Based on the physician assistant’s admission that he is impaired in his ability to practice in accordance with acceptable and prevailing standards of care because of habitual or excessive use or abuse of drugs, alcohol, or other substances that impair ability to practice; and that, he committed an act that constitutes a felony in Ohio, regardless of the jurisdiction in which the act was committed; and that, he made false, fraudulent, deceptive, or misleading statements. Agreement effective 12/13/2017.
