05/11/2022 BOARD ORDER - Application for restoration of license to practice as a physician assistant permanently denied. Based on the submission of an application for restoration of license to practice as a physician assistant, and on the finding that the physician assistant’s pleas of guilty in the Fayette Circuit Court, Indiana, to Battery, a Class A Misdemeanor, and Forgery, a Class C Felony; and on a prior action by the Indiana Physician Assistant Committee, which issued a 3/9/16 Proposed Settlement Agreement and a 4/4/16 Final Order, which individually and/or collectively constitute: a plea of guilty to, a judicial finding of guilt of, or a judicial finding of eligibility for intervention in lieu of conviction for, a misdemeanor involving moral turpitude; and a plea of guilty to, a judicial finding of guilt of, or a judicial finding of eligibility for intervention in lieu of conviction for, a felony; and also any of the following actions taken by the state agency responsible for regulating the practice of physician assistants in another state, for any reason other than the nonpayment of fees: the limitation, revocation, or suspension of an individual's license to practice; acceptance of an individual's license surrender; denial of a license; refusal to renew or reinstate a license; imposition of probation; or issuance of an order of censure or other reprimand.
