Application Instructions

Eligibility for a Certificate to Prescribe (CTP) or Certificate to Prescribe Externship (CTP-E) requires the possession of a current, valid Ohio RN license and a Certificate of Authority (COA) to practice as a Certified Nurse-Midwife (CNM), Certified Nurse Practitioner (CNP) or Clinical Nurse Specialist (CNS).

If you do not hold prescriptive authority in any other state/jurisdiction or you hold/held prescriptive authority in another state/jurisdiction for less than a continuous year within the past three years, you are eligible for a CTP-E (1500 hour externship).
If you have/held authority in another state/jurisdiction to prescribe drugs and therapeutic devises excluding controlled substances for at least 1 continuous year within the past 3 years, you are eligible for a CTP-E (500 externship hours).
If you have/held authority in another state/jurisdiction to prescribe drugs and therapeutic devises including at least some controlled substances for at least 1 continuous year within the past 3 years, you are eligible for a CTP.

CTP-E (1500 HOURS) - EXTERNSHIP
Steps to obtain a CTP-E (1500 hour) Externship

1. A $50 non-refundable fee must accompany this application and will be processed electronically

2. Documentation of advanced pharmacology completed within the past 3 years.
If the advanced pharmacology course is over 3 years old when the CTP-E application is received, another 45 contact hour advanced pharmacology course must be taken. Upload documentation of your advanced pharmacology education in the form of a photocopy of a school transcript, certificate or grade report. Advanced Pharmacology Course of Study Hour Breakdown
a. At least thirty-six (36) contact hours must be obtained from a single provider in pharmacokinetic principles, clinical application; principles of the use of drugs and therapeutic devices in the prevention of illness and maintenance of health; 
b. fiscal and ethical implications of prescribing drugs and therapeutic devices;
c. the state and federal laws that apply to the authority to prescribe; 
d. instruction that is specific to schedule II controlled substances (schedule IIs), including instruction in indications and contraindications for the use of schedule IIs in drug therapies, the most recent guidelines and recommendations for pain management therapies and education, and stimulant therapies, fiscal and ethical implications of prescribing schedule IIs, state and federal laws that apply to the authority to prescribe schedule IIs and prevention of abuse and diversion of schedule IIs. Instruction specific to schedule IIs may be integrated with areas of instruction as indicated above in a, b, and c

3. Your CTP cannot be issued until you submit collaborating physician information.

4. Upon completion of Externship hours, Form B should be submitted to the Board. Form B is located on the Board's website. 

CTP-E (500 HOUR) EXTERNSHIP
Steps to Obtain a CTP-E (500 hour) Externship

1. Non-refundable Application Fee
A $50 non-refundable fee must accompany this application and will be processed electronically 
 
2. Verification of prescriptive authority in another state/jurisdiction.
Complete the first page of Form A and forward both pages to the other jurisdiction or federal government to provide verification of your prescriptive authority.

3. Your CTP-E cannot be issued until you submit collaborating physician

4. Upon completion of Externship Hours, Form B should be submitted to the Board. Form is located on the Board's website.  

CTP (NO EXTERNSHIP REQUIRED)
Steps to obtain a CTP
1. Non-refundable Application Fee

A $50 non-refundable fee must accompany this application and will be processed electronically. 
 
2. Verification of prescriptive authority in another state/jurisdiction.
Complete the first page of Form A and forward both pages to the other jurisdiction or federal government to provide verification of your prescriptive authority. You can download Form A from the Board's website. Click on the "Forms and Applications" link on the Board's homepage. The information is provided under the Advanced Practice Nursing Section
 
3. Completion of two (2) contact hours in Ohio laws governing drugs and prescriptive authority.

COA’s (CRNA, CNS, CNM, CNP) and CTP’s that are newly issued from July 1 – Oct 31 of the RN renewal year (odd year) will expire on October 31 of that year. You will be required to renew your certification prior to October 31, however, you will receive a first-time renewal fee waiver (no fee will be charged).

Your social security number is required by state and federal law for purposes of child support enforcement (ORC 3123.50, 42 U.S.C. Section 666), reporting to the National Practitioner Data Bank (Public Law 100-93, Sec. 1921 of the Social Security Act, as amended; 45 C.F.R. pt. 60); reporting to law enforcement authorities for investigative/law enforcement purposes in compliance with ORC 4723.28, and/or as otherwise required by state and federal law.
