Upgrade a License From Temporary to Full


The license upgrade application applies to any individual who holds temporary orthotics, prosthetics, or pedorthics license in Ohio and who wants to obtain a full license. TO APPLY Applicants are required to electronically upload documents to apply for licensure. The application fee is non-refundable and must be paid by credit card (Visa, MasterCard, or Discover) or electronic check. Failure to supply required information may result in denial of the application. If your application remains incomplete for one year from the date it is received, your file will be closed. Providing your Social Security Number for licensure is required by state and federal law. I hereby authorize all my references, educational institutions, employers, business, professional organizations, and associates - past, present, and future - to release to the Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board any information requested by the Board in connection with the processing of this application or subsequent licensure.

I hereby certify to the Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board that I will not use any initials, titles, or words which imply that I am licensed in Ohio to perform services until I am granted licensure by the Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board.

I further certify that if I accept employment prior to licensure by the Board, I will only perform duties that may be legally performed under my current license.

I further certify that if I am not a U.S. citizen or permanent resident, I am not employed in any capacity that violates the provisions of my alien/non-immigrant status.

I understand that the Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board is authorized by law to initiate action against a person who practices or offers to practice orthotics, prosthetics, or pedorthics, or use in connection with the person’s name, or otherwise assume, use, or advertise, any title, initials, or description tending to convey the impression that the person is an orthotist, prosthetist, or pedorthist if that person does not hold a current license under ORC 4779.

I further understand that pursuant to ORC 4779.28, the Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board may suspend, revoke, or refuse to issue or renew the license of an individual who has violated any of the laws and rules governing the practice of orthotics, prosthetics, or pedorthics in the State of Ohio.


Consolidate to a Licensed Prosthetist-Orthotist


The license consolidation application applies to any individual who holds an active orthotics or prosthetics license in Ohio and who wants to obtain an prosthetist-orthotist license. TO APPLY Applicants are required to electronically upload documents to apply for licensure. Some documents are required to be sent directly to the Ohio OTPTAT Board. The application fee is non-refundable and must be paid by credit card (Visa, MasterCard, or Discover) or electronic check. Failure to supply required information may result in denial of the application. If your application remains incomplete for one year from the date it is received, your file will be closed. Inactivate My Certificate/License

Individuals may put their renewable certificate/license in Inactive, Escrow status for up to five (5) years for medical reasons, moving out of state or leaving the field for a period of time. The non-refundable fee is $75.00.

Escrow status should not be used as an extension to complete CEU renewal hours as there is a one (1) year grace period to renew with a late fee.

Individuals must RENEW before the expiration date of escrow status. Individuals may renew anytime within the five-year escrow period by meeting the renewal requirements and paying the renewal fee. Failure to renew before escrow status ends will result in permanent expiration of this license/certificate.

NOTE: Individuals cannot practice under an inactive certificate/license.
